Patient Outreach Specialist Job Description
Patient Outreach Specialist are responsible for patient outreach including but not limited to: Connectivity, No-Shows, No-Monitor Patients, New monitor set-up and any outreach associated with calling or contacting patients, including but not limited to text messages, mailing letters and phone calls. Secondarily responsible for creating/maintaining patient schedules as needed. May also be responsible for working in client’s EHR and other workflow systems.
Communicate with patients, RMG staff and/or clients regarding, but not limited to:
- No Shows
- Connectivity
- Monitor Set-Up
- Monitor Orders
- Transmission Requests
- Reminder Calls for
- Any request regarding or requiring patient
Communication methods include, but is not limited to:
- Phone Calls
- Emails
- Text messaging
- In-person Verbally
Create & Service cases in Salesforce including, but not limited to:
- No Shows
- Connectivity
- No Monitor/Monitor Pairing
- General Admin Requests
- Clinical Requests
- Medical Records/Registration
Perform client EHR Navigation as needed, for ANY task associated or required to be completed within client EHR system.
Job requires acceptance of departmental transfer to other service areas as needed or dictated by Rhythm Management Group, including but not limited to: Patient Communications, Health Information, Client Systems, Clinical, and RPM.

Education and background
